The Dog Lodge Application
The Dog Lodge application form is required for daycare and boarding. You will be given this application to take home when you bring your dog in for a personality test. If you are certain your dog will do well in this type of environment and does not need a test, you can download and print this form and bring it in when you drop off your dog. Otherwise plan to spend a few extra minutes to fill it out when you arrive.
